Q:  How do I order a birth certificate? Death certificate?
A: 
Births and deaths occurring in the city limits of Findlay are filed in the Findlay City Health Department. A certificate may be purchased from 8:30 a.m. to 3:30 p.m., Monday through Friday, at the Health Department for $25 per copy. Cash and checks are accepted.   When calling the Health Department concerning  Birth Certificates or Death Certificates questions at 419-424-7105, ask for Vital Statistics Division.
Applications for Birth Certificates or Death Certificates along with instructions for purchasing the certificates by mail, are available on the Health Department website. Q:  Do I need an appointment to get shots for my child?
A: 
Yes—have paper and pencil ready, and call 419-424-7441 between 8 a.m. and 5 p.m., Monday through Friday to schedule an appointment. More information on immunizations 
 
Q:  I want to sell baked and/or homemade food items. Do I need a food permit?
A: 
Probably—Permits are usually required, depending on the type of food to be sold, the setup being used, and the frequency of the operation. Q:  When do I need to get a plumbing permit?
A: 
Permits and inspections are required for all residential work except repairs in Findlay and when you install a new water heater, furnace, and add or move plumbing. These forms are available on our plumbing webpage. Ask your plumbing contractor if he is filing for you
 
Q:  Can I get a flu shot?
A:  The health department follows CDC’s recommendation that flu shots go to “priority groups” first as indicated below: 
To ensure that those who are at highest risk of complications from influenza have access to vaccine, CDC recommends that people in certain priority groups receive inactivated influenza vaccine:
 
¨      People aged 65 years and older, with and without chronic health conditions
¨      Residents of long-term care facilities
¨      People aged 2–64 years with chronic health conditions
¨      Children aged 6–23 months
¨      Pregnant women
¨      Health-care personnel who provide direct patient care
¨      Household contacts and out-of-home caregivers of children less than 6 months of age
¨      A date will be announced when vaccine is available for all persons, regardless of age or health status.
